Presence of donor specific HLA class 2 antibodies (DSA class 2) is associated with development of graft fibrosis more than 10 years after liver transplantation–a retrospective single center study

چکیده
Here the impact of donor specific human leukocyte antigen (HLA) class 2 antibodies (DSA cl 2) on long term outcome after liver transplantation (LT) was investigated. Altogether 156 (44 pediatric and 112 adult) LT recipients were included in study. Graft fibrosis assessed by elastography biopsy. DSA determined Luminex technology. 46% positive for a median follow-up 15 years. In multivariate analysis significantly associated with immunosuppressive monotherapy (OR 5.42; 95% CI: 1.02–28.90; p = .048). Compared to negative patients, had more graft based stiffness (mean 9.4 ± 9.0 kPa vs. 6.5 6.3 kPa; < .002) stages (p .016) performed biopsies .002). Also, higher incidence chronic rejections (11% 2%; .045) losses (6% 0%; .043) found. regression 4.57; CI 1.59–13.10; .005). So, these data suggest that development occurs often may ultimately result rejection fibrosis.

Long-term Clinical Relevance of De Novo Donor-Specific Antibodies After Pediatric Liver Transplantation.
BACKGROUND Anti-HLA antibodies and especially donor-specific antibodies (DSA) play a significant role in graft survival after solid organ transplantation. Their impact on long-term survival in adult liver transplantation (LT) is controversial, but they may be a risk factor. The effects of DSA after pediatric LT are still unclear. METHODS We performed a retrospective evaluation of DSA in sera ...

متن کاملEffect of donor-specific antibodies and panel reactive antibodies in living donor liver transplant recipients
PURPOSE Preformed circulating donor-specific antibodies (DSAs) immunologically challenge vascular endothelium and the bile duct. However, the liver is an immune-tolerant organ and can avoid immunological challenges. This study was undertaken to analyze the effects of DSAs after adult living donor liver transplantation (LDLT). METHODS We retrospectively reviewed 219 LDLT patients' records trea...
متن کاملEverolimus Is Associated With Less Weight Gain Than Tacrolimus 2 Years After Liver Transplantation: Results of a Randomized Multicenter Study
BACKGROUND Weight gain early after transplant is a risk factor for posttransplant metabolic syndrome (PTMS), cardiovascular events, and renal insufficiency. The impact of mammalian target of rapamycin inhibition on posttransplant weight gain and the development of PTMS components postliver transplantation were examined in a randomized, controlled study. METHODS After a run-in period, patients...
متن کاملAnti-HLA Antibodies and Kidney Allograft Outcomes in Recipients with Donor Bone Marrow Cell Infusion
Background: Anti-HLA-antibodies are known to affect the allograft survival in transplant recipient patients. Objective: The aim of this study was to evaluate the association between anti-HLA antibodies and kidney allograft outcomes, particularly in recipients with concur-rent donor bone marrow cell infusion (DBMI). Methods: Between June 2006 and May 2007, forty living unrelated donor kidney tra...
